Financial Opportunity Center (FOC) Intake Packet
PLEASE READ BEFORE STARTING THE APPLICATION
- CCC FOC Intake Packet is available online. The Intake packet and required supportive documents can be completed and submitted online via JotForm, an online platform. Please read the instructions to upload your files on JotForm HERE.
- You must attend orientation prior to completing the Intake Packet. You can register for FOC orientation HERE or to learn more about Financial Opportunity Center (FOC).
- You must complete the Intake Packet in one session.You may not save and return to your open Intake Packet. Closing an active intake before submitting will result in loss of information and you will need to restart the intake to ensure that we receive the necessary documents needed to process your request.
- Please have the following documents ready before you begin. You can upload the completed documents on Page 28 of the Intake below. Phone images and scans are acceptable.
- Valid Driver’s License or State ID
- Social Security Card
- Proof of at least a high school education (High School Diploma) or GED
- Proof of Income: Acceptable documents includes:
- Paystubs
- Payment in lieu of earning including unemployment or disability – verify amount received or use prior year tax return if not received
- Self-employed: current year-to-date net income or prior year tax return or average 2-year payment amount
- Periodic payments from SS, annuities, insurance, pensions, etc.
- Welfare assistance with any adjustments
- Alimony
- Child Support
- Zero income Enrollment Certification
- Self-Certification Form
