Financial Opportunity Center (FOC) Intake Packet

PLEASE READ BEFORE STARTING THE APPLICATION
 
The CCC FOC Intake Packet is available online. You can complete and submit the Intake Packet and required supporting documents via JotForm, an online platform.

You must attend orientation before completing the Intake Packet. You can register for FOC orientation, or learn more about the Financial Opportunity Center (FOC).

The Intake Packet must be completed in one session. You cannot save and return to an open Intake Packet. If you close an active intake before submitting, your information will be lost, and you will need to restart the intake process to ensure that we receive the necessary documents to process your request.

Please have the following documents ready before you begin. You can upload the completed documents on Page 28 of the Intake Packet below. Phone images and scans are acceptable.

• Valid Driver’s License or State ID
• Social Security Card
• Proof of at least a high school education (High School Diploma) or GED
• Proof of Income: Acceptable documents includes:
  • Pay stubs
  • Payment in lieu of earnings (including unemployment or disability) – verify the amount received or use the prior year’s tax return if not received
  • Self-employed: current year-to-date net income or prior year tax return, or average 2-year payment amount
  • Periodic payments from SS, annuities, insurance, pensions, etc.
  • Welfare assistance with any adjustments
  • Alimony
  • Child Support
  • Zero income Enrollment Certification
  • Self-Certification Form
